Dear Medicare Beneficiary,
We are writing to let you know that your doctor is part of NPO-CIN, a Medicare Direct Contracting Entity (DCE) participating in a program within Medicare.
Your Medicare benefits have not changed. Your doctor asked NPO-CIN to help see that you get the right care at the right time. You still have the freedom of choice to go to any doctor, hospital, or other healthcare provider of your choice that accepts Medicare.
A DCE is a group of doctors, hospitals, and other healthcare providers who agree to work together to keep you healthy. All members of NPO-CIN agree to work together to see that you get the right care at the right time. We will help everyone work together to give you better care. We will work to support your Primary Care Physician to help coordinate your care according to your individual medical needs and treatment choices. We will protect your medical records and privacy. We will work to reduce duplicate tests and duplicate paperwork that cost you time and money. To see a list of the doctors and other groups that work with us, please see attached list.
NPO-CIN is confident a physician-led organization can make a difference and improve the overall health of our community by focusing on patient-centered care and lowering costs by improving communications between providers. This will decrease duplication, improve cost transparency and provide you a choice of services, improving your experience and continuing to improve patient outcomes.

Northern Michigan Health Network doing business as NPO-CIN has been in existence since 2013 and participating with the CMS Medicare Shared Savings Program from 2014-2019.
Northern Michigan Health Network was formed by a group of physicians in Northern Michigan to demonstrate that a strong partnership between primary care providers and patients can lower healthcare costs and improve the patients’ outcomes.
